you can set up sound icon for capital letters
in /etc/speech-dispatcher/speechd.conf
See the paragraph in which it is discribed. It works in all cases,
unlike my former statement.
You may try to set up the voice variant in the same file by choosing
under LanguageDefaultModule your language +... but I don't know whether
that works; haven't tested it, cause I doon't like this variants.
