Would be good to check debug logs from ModemManager. Can you follow 
these steps to get them?

$> sudo mv /usr/sbin/modem-manager /
$> sudo killall -9 modem-manager
$> sudo /modem-manager --debug
<now try to reproduce the issue>

Then move back the binary to the original place:
$> sudo mv /modem-manager /usr/sbin
