I had to choose 'Default' instead of 'Intel 82801DB-ICH4' for both audio
devices in the GM settings to make it work with esd.

But one minor problem remains in this setup:
If 'Default' is chosen for both devices the incoming audio data has a
delay of about a quarter to half a second while the outgoing data seems
to have no delay. The jitter buffer slowly increases to its maximum
allowed value of 500 ms. This is independent of esd running or not and
can not be observed if the Intel devices are chosen instead of Default.
