Agreed. That's the same issue as for other kinds of extensions (Firefox,
OpenOffice...). It would really be nice to find a solution to integrate
better with package management systems to avoid the mess of each app
upgrading extensions and annoying users with different upgrade dialogs
(plus instability...).

But I'm not sure you can fix this problem alone. ;-) addons.gnome.org is
an interesting idea, and the Shell extensions could integrate into that
framework, it if is eventually implemented.
