Are you familiar with gnopernicus, the screenreader which is part of
GNOME 2.4? I don't know if it currently does what you're trying to do,
but it might be worth checking the source code for hints.
You might also want to join gnome-accessibility-list gnome org, where
you'll find some people who are very enthusiastic about making GNOME
accessible to blind (and other) users :)
