I hope you meant that additional file management features have to be put
in a context menu?

Else, if you're advocating that the "create directory", "rename" and
"delete" operations shouldn't be buttons, but just hidden away in the
context menu, you obviously didn't read the criticism of the win95 file
dialog.

Certain "clutter" is sometimes necessary to give the user a hint that he
can actually do those things. That's why buttons are for, and we don't
use context menus for everything, except for operations that can be
performed without knowing about the context menu (that's the main
usability issue with the Gimp, a new user won't know that everything is
hidden in context menus).

I think that "create directory", "rename" and "delete" are minimal and
important operations that should be performable inside a save dialog,
and as such, those operations should be visible with buttons.
