Or it indicates the file manager is not an intuitive, fast, simple,
accessible tool.

If it can be reached quickly (from within the dialog, since that adds the
possibiliy of opening it with the file dialog's current folder as the
startup folder), starts up immediately and is simple to use this will almost
completely eliminate the need for in-dialog filesystem management, IMHO. The
only options that should be kept are to create a new folder and to rename
and delete existing files. Note that since renmae and move are the same, you
can also move files with only these three commands.
