Will Walker of the orca team seemed to think the problem was in Speech Dispatcher itself (as of last week). Perhaps you would contact him about it?

Not really.  This limitation only applies to the current Orca backend
for Speech Dispatcher.  Speech Dispatcher itself supports "index marks"
which are exactly for this purpose.
