Major difference is I'm using RHL9/XD2 and you're using Mdk9.1.

Ditto.

 Terminal Font: Luxi Mono, 8pt

*However* on RHL9/XD2 there is a bug which means that GtkHTML doesn't
pick up on what the default GNOME Terminal Font is (mine's set to Vera
Sans Mono 10pt), and as a result seems to use the system default
monospace font as defined by fontconfig (which in this case is Luxi
Mono). That bug has already been reported.

Printing issue: Uses the first font in alphabet. Seems unrelated.
Gnome default: Not sure, feels unrelated too, IMHO.

I'm not sure about the latter. They *might* be related.

The big question is: *How* can fonts be rendered different? The reason
seems even not to be related to the font (as we use the same).

Yes, it certainly is a strange one.

Best, Darren
