[Evolution] ldap config issue

When I attempt to add an additional LDAP server to the list of
directories, the "OK" button is greyed out.  I've tried a number of
permutations for filling in the configuration screen, and none have
enabled the OK button.

If I attempt to edit another entry (Bigfoot for example) it kills the
addressbook.

Anyone have any suggestions?

I'm running 0.9 +cvs.2001.03.07.09.05  (updated through RedCarpet) &
openldap-1.2.11-15
