Apparently the problem was introduced recently since I can not
reproduce
using version 40.0.
Steps to reproduce:
1. Launch the latest version of orca.
2. Open a file in VSCode-insiders.
3. Navigate in the files using arrows.
4. Change to another window using alt+tab.
5. Change back to VSCode windows using alt+tab.
6. Try to navigate in the file using arrows.
At least in my environment nothing is spoken by orca.
I need to press alt two times to restore the normal behavior.
