If that might be more comfortable to you than browsing the menus, you can move focus to somewhere else besides message body document e.g. to the subject, to or cc fields and pressing shortcut keys will work there.

This is unfortunately a known issue for a long time, see https://bugzilla.mozilla.org/show_bug.cgi?id=269228
As stated by Jean-Philippe, it is not a bug related to Orca, it also happens without Orca.
What I do to add an attachment is:
- Press alt + f to open the file menu
- Then the letter j to open join menu (I don't know the exact translation, I'm on a French system)
- then enter

Hello everyone, When composing a message in thunderbird using orca, I try to press shortcuts like ctrl+shift+a to attach the file, but I don't have any success. Instead of opening the dialog orca selects the text. However, if I press alt+f and in the attach menu choose the send file option there is the same shortcut.
