I don't know about speech as I rarely have it turned on but I know with
braille and the default installs of orca and thunderbird on ubuntu 20.04 I
do not see on the display what I am typing and in fact I don't know
exactly where the focus is. I can see my message by going out of the
email I am typing and coming back in.
Not a serious issue as I only use thunderbird for a couple of lightly used
email accounts and do most of my email with alpine.
