I also think that something has already been reported on this, but
honestly I haven't found anything on the VSCode issue list.
Would it help if I sent a debug file reproducing the problem?
Anyway, I'll keep looking to see if I can find something.

1. Launch VSCode.
2. Press ctrl+shift+p.
3. Type only the letter a.
Orca announces the name of a VSCode command, make a note of that
command.
4. Press the down arrow a few times to select the next commands.
In my environment, before announcing the next command, orca always
announces the command presented in step 3.
