While testing I noticed that sometimes Orca doesn't say the new
document when you switch between page tabs. When this occurs, it
appears to be due to a VSCode issue (telling us the document you were
in just got focus). I'll look into that further and then report it to
them.

Not sure if the problem is with orca or VSCode.
Steps to reproduce.
1.Launch VSCode insiders.
2> Create an empty file by pressing ctrl + n.
3. Go to extension view pressing ctrl+shift+x.
4. Type python.
5. Press tab and choose the first extension in the list using
up/down arrow.
6. Press enter.
A virtual document with the description of the extension must be
displayed and we can use the arrows to navigate it.
7. Change to another file pressing ctrl+tab.
8. Go back to the virtual document created in step 6 using ctrl + tab.
9. Try to navigate the document using the arrows.
Nothing happens. It is necessary to switch to browse mode in order
to navigate the document with the arrows.
