Update: I'm not sure what is common to the menu items which are
presented by Orca; - it isn't the presence of shortcut key definitions.

The only indication of a possible cause I can find in the debug log is
10:50:45.563418 - INFO: [menu | ] reports 0 selected children
Can others reproduce? I can provide more detailed logs if desired.
