Here is my pkgbuild script with Carlo's commit included.

Unfortunatelly I have just found even more edge cases.

For example when opening popup menus and exiting out of them the focus is not restored back to the application. Or better to say the focus is managed correctly however orca is not seeing that.

I've also commented at https://gitlab.gnome.org/GNOME/gnome-shell/issues/452#note_320264

Btw Carlos Garnacho has prepared a merge request to mutter which appears to fix the issue for gnome 3.30 at least from my initial testing.
For Arch linux users: I'm not in front of my machine right now but if you are interested I can share my pkgbuild script in a hour or so.

Yes, that's the package that introduces the change, so it's not
necessary to downgrade everything, just gtk.
