I jut downloaded and installed elementary 0.4.1 in a VirtualBox VM.

That's actually a refurbished Ubuntu, lacking accessibility during installation and usage out of the box.

This is true for the default login manager and desktop Pantheon.

You have at least some a11y apps installed like gnome-orca and speech-dispatcher but you'd need to install 
and set additional components from an Ubuntu repo and replace the desktop to get accessibilty.

Not worthwhile  IMHO, better just install Ubuntu Mate directly if you like it.

There are a few specific apps, but that don't bring much added value in my opinion.
