Hi,
Yeah, I had the same issue myself, especially since Orca will stop
running once in a while. Again, just create a luncher on the desktop
that executes the command: /usr/bin/orca -reload
For example, I know when Orca turns off on me, I hit Ctrl+Alt+D to
return to the desktop, press the Up arrow key a bunch of times to
ensure I'm at the top, then Orca is two Down key presses down to
restart it. I don't know, but works for me at least.

auto-start but applications use it to allow to find them, for example
Firefox has a desktop file to allow to find it from the "internet"
category of the Mate menu or from the research field of GNOME.

Uncertain what OS you're running, but on Linux Mint MATE at least,
this is very easy. I just press Ctrl+Alt+D to get to the desktop,
then I use my laptop touchpad to click anywhere on the screen. Then I
push the * button on my keypad to emulate a right mouse click, which
opens the context menus, and allows me to create a new launcher. You
need to know the command line argument though (eg. Orca is
/usr/bin/orca -reload)

IMO, we should provide a desktop file to Orca. On Windows, both Jaws,
NVDA and the narrator are discoverable by the start menu. I see no
reason that make the end-user more happy without desktop file than he is
with.
I can figure out what commit drop desktop file and tell you if you want.
