Hey Alex.
I'm working on the first issue as that is in Orca.
Regarding the second issue, if you find the object in Accerciser it's a
link with accessible name "user avatar." That link has a child with the
name "user avatar". I don't see any descriptions or other means of
getting at the title. Perhaps the alt tag is stomping on it? I thus
think that will need to be fixed elsewhere.

1) Go to https://gitlab.gnome.org/GNOME/orca/issues/7
2) Login if it's not already the case
3) Search for "Notifications"
4) Read the line with up/down arrow
Result: Orca says "Notifications image"
Expected result: Orca should say Notification toggle status on (or off)"
5) Press tab to go to the button
Result: Orca announces "Toggle button on"
