try raising the volume after plugging in or unplugging your head phones.
If it's the bug I think it is, it has nothing to do with Orca or your
TTS. It's somewhere in the audio stack.

I am not sure if trying to do an ALT Super S at the loggin screen will
resolve this bug like it did when I first had ubuntu installed on this
computer when I had to work out if I was at the loggin screen before
turning on Orca.
