Hello,
Perhaps at-spi has crashed. Can you verify if it's running?
The easiest way on how to bring it back is by running
Orca ---replace

Some thimes orca refuses to start.
I tried running it in a terminal an got following messages:

** (orca:30554): WARNING **: Couldn't connect to accessibility bus: Failed to connect to socket /tmp/dbus-eLGseGwTM0: Verbindungsaufbau abgelehnt

** (orca:30554): ERROR **: AT-SPI: Couldn't connect to accessibility bus. Is at-spi-bus-launcher running?

I know after a rebot it might  work again but what cuases the error here?
Ubuntu 16.04.
