I used to have Orca stop speaking some times with firefox in
the early days of 16,04.
But i used to press alt+ctrl + t and then in press the Orca + s
2 times and after a couple of seconds speach would come back.

including orca from the accessibility PPA.
He told me that sometimes, when using firefox, orca stops as
if someone or something was killed orca. The message 'screen
reader off' is announced by orca and after that orca stops
completely.
I received a debug file from him but I didn't find a
possible cause. It seems that orca was killed by an external
process. In the debug file I found the information about
orca is going down.
I know that this is strange and I don't have much
information. I am writing to the list in the hope that
someone can help in some way.
