Unfortunately, there is a problem in all these cases.
There is only one situation that I could not check. If you start at the
beginning of "firefox", and then "orca", the "firefox" window for "orca"
is not fully available, ie "Orca" in the window "firefox" is silent.

Otherwise, I need to figure out how to reproduce it.
But if that bug is what you're experiencing, then the good news is that
I have come up with a means (still in progress) to work around it. The
bad news is that it's a pretty significant change -- one I am hesitant
to commit this close to the GNOME 3.20 release because of concerns about
regressions. However, once I branch for the stable release, I can commit
the change to Orca. And once Mozilla fixes their bug, users of stable
Orca will see the problem go away.
