[orca-list] Flat review issue in menu items

I've noticed several issues with flat review in menu item.

Test environment :
- Orca 3.19.91pre (updated from master)
- GNOME Shell 3.18.3
- Mate 1.12.1

Steps to reproduce in Thunderbird :
1) Open Thunderbird / Terminal (Gnome) / LibreOffice Writer
2) Press F10
3) Press down arrow
4) Press orca+i to read current flat review line, it fails and read the menu bar, not the current selected menu item

I've noticed the same issue with the Mate menu (maybe "application")
