In my opinion it is normal thatOrca speaks the character right of the caret. When I type 12345 and press the home button Orca says 1. Pressing right arrow Orca will say 2.

To clarify: If I have any text editor open and type 12345 when I go back to the beginning of the line and work my way with the right arrow button the first number it would speak would be the 2. When I use the left arrow button it says the character it just passed.

That behavior really confuses me when fixing typeos cause I always have to check again if the inserted or deleted letter was on the right position.
