Ya guys mentioned Facebook Messenger in here somewhere, ya may wish to check out the:

stand-alone web client.

Works very well with NVDA on Windows, thus should work nice with Orca.
With Chromevox, its a bit odd with it, but it can be gotten around fairly well.

My fav service to use, especially since I use an Acer Chromebook 11, is:

, both  Messenger and Appear use Web RTC, and work on Firefox and Chrome.

Support for Audio/Video for the web version of Skype:

, won't be coming till next year, if you're on Linux/Chrome OS, though IM is supported  of course.

Lastly, Google Hangouts indeed is accessible, and too has its  own stand-alone client now:

So, quite a few means of going  with the Voice Over IP way of communicating.
