If you can hear "Screen reader on" message just after you login into gnome, then I assume one little think is happening. In this state nothing has the system focus thus pressing any keys e.g. ctrl key, orca+t for date time, any letter keys will produce no speech output from orca since at this time orca is not yet notified of these keypresses. To fix that just press your super key also known as the windows key or alt+F1 keyboard shortcut. As you press this you should hear orca saying "Window" and from here you can work as normal. You have just expanded a gnome overview so you can for example start typing part of your keyword e.g. thu for thunderbird, ge for gedit and similar. Or you can press ctrl+alt+tab in order to land so called top pannel where you can move left to right between sections and up / down inside each section. E.G. modern gnome apps display their menubar in this top pannel, there is calendar and all the so called pannel indicators including volume control, network connections, session controls and similar.

Hopefully this is your situation as this is really easy to pass over as long as you know it.

From my experience this is a thing which is not a big deal for long time linux users but is verry likelly to piss new comers off.

Your problem is with speech dispatcher.  You have to tell it that you are
using alsa as the sound output and not pulse audio which is the default.
 From your console session with Speakup, type spd-conf and press enter.
Then, follow the prompts for a user-specific setup.  When you get to the
sound output question, type alsa and then hit enter.  Proceed through the
remaining options.  Once you are done, orca should talk in Gnome.

I installed Debian Linux some Days ago.
There are no sounds at all in gnome.
I didn't want pulse audio, so I removed that.
I can get alsa to work in the console.
But not in gnome.
There was no sound when I had pulse audio installed either.
I thought that the problem was orca so I ran orca --text-setup.
The only message I got was the question if braille should be activated.
That was the only question.
Shouldn't there be more?
At least it was Before.
What can be wrong here?
