i know about this problem,
it seems that speech-dispatcher goes also down.(it seems that speechd has many problems at the moment, i have also a view problems ) i wrote me an script to restart orca with an pkill speech-dispatcher first.
you can ad custom scripts to the keyboardshortcuts. maybe this helps

I am using orca in debian wheezy. It sometimes stops responding to orca keystrokes So if I press insert+space, nothing happens. The worst is that pressing control to interrupt speech doesn't work. Restarting orca with "orca --replace" doesn't fix the problem. Is there some other way to restart orca so that it starts accepting keystrokes again?
