I am trying to reproduce this with Firefox 25 and Orca master.
I can't make orca to freeze however while looking at the page source of the article you've linked to I can see the following: <p>The next part is: <code>group by value.group</code>. This will tell Angular.js that it should make up <code><optgroup></code> tags and the label attribute will be filled with the content of the group field.</p>

The first <code></code> is reported as text and the second one is reported as pannel with no content while arrowing.

The solution is restart orca.
