Gnome/Unity :
1 - press super key
2 - type orca
3 - press enter
4 - now, enable for all logins in gnome-control-center

XFCE/LXDE/KDE:
1 - presss ALT+F2
2 - type orca
3 - press enter
4 - put orca in your autostart file

just a WM(no DE):
1 - open your run dialog
2 - type orca
3 - press enter
4 - put orca in your autostart file

if you start X from the CLI:
put orca before your DE/WM start commands in ~/.xinitrc

Docs are quite good and inster+h is a good thing too. Insert+h puts you in "learner mode" where you can explore the different key strokes by pushing
them and learn what they do.
