I would say that, at present, GNOME is the most accessible GNU/Linux desktop; by GNOME, I include its variants like Unity-2D. The QT accessibility bridge makes a few applications usable, in my experience, but the full KDE desktop is not; if I'm mistaken, someone please enlighten. Jonathan Nadeau is experimenting with XFCE on an installation of XUbuntu 12.10. If this proves to be a practical alternative to GNOME 3, it would be a light-weight option for those who don't want GNOME 3. Also, Knoppix Adriane offers some accessibility to one of the light-weight desktops, along with its own screen reader, SBL and Adriane. Regarding a centralized, distribution-independent knowledge base for this kind of information, there is one in the works. Have a look at http://accessiblefreedom.org/wiki/ where we've only just begun.
