The alt-key-above-tab is used to switch between windows in the same
application. The alt+esc key is used to switch between all windows.

I suspect it isn't a bug.
To switch between applications, use alt-tab. To switch between
windows, use alt-key-above-tab, i.e., alt with the key above the
tab key, whatever that is - on every keyboard I am aware of, it is
not the escape key.
