Have a look at http://live.gnome.org/Orca/AccessibleLogin. While debian
isn't specifically mentioned I would imagine that it won't differ too
much from some of the other distributions. I think those instructions
has orca starting automatically, I don't know what would be done to give
it a key stroke that you can use to start orca.

I did find the instructions in the Gnome accessibility guide, which apply to
Gdm 2 only - the configuration has changed for Gdm 3.
