Quite a bit has changed since then, including in Orca and in AT-SPI2.

ongoing efforts to solve this problems?

Yes, many of them are in Orca 3.2. A bunch more are in 3.3.3, and even
more are in master.

In a nutshell, our problem is what I call "Dead Accessibles." Objects go
away, but we still have an accessible reference to them. When we attempt
to get information about that object which no longer exists, bad things
happen. Like application crashes. As a result, I have added sanity
checks throughout Orca to ensure this no longer occurs. I could have
easily missed one or two, but I think you'll find the latest Orca
considerably more stable.

Similarly, much work has been done in AT-SPI2 by Mike Gorse since 10.10
to make it much more reliable.

Hope this helps and sorry for the troubles you are experiencing!

Take care.
