I think the short answer is "Yes."  Orca now works well with
PulseAudio through speech-dispatcher.  However, there are still
various complaints.  Sometimes multiple instances of pulseaudio wind
up running at the same time, and that can cause sound to lock up.
Older games sometimes have long latencies, of about a second, making
them unplayable, because pulseaudio has a very high default latency.
But if all you need is Orca working well, Ubuntu Lucid has it.
