I never heard "??" at any time before or after the change.  But to
answer Bill's part below, I didn't give key echo any thought because I
was mainly addressing text being spoken when you cursor over the
document.  I usually use echo by word when typing; I'll have to
checkout echoing of keystrokes by char the next time I'm in Orca.  I'm
doing this mail from the console right now so my results vary.

I still get somewhat goofy behavior I can't explain after the patch.
With Key Echo enabled, now both espeak and voxin say "capital" as I
type.  It was pretty annoying, but when I turned off key echo, and
enabled "echo by character", it seems to work ok.

Yeah, there might be some other fall-out with character substitution.
I was trying to get the speech dispatcher modules to behave more like
gnome-speech since it seems that gnome-speech does a better job with
substitution of characters. See, formerly in the speech dispatcher
modules, character pronunciation is left up to the synthesizer. In
the case of the bullet styles used in Open Office, the synth did not
have descriptions available for at least one style I know of for
sure. So I examined the gnome-speech factory module and took out what
I needed. There was a lot of extra stuff, it seemed, in gnome-speech
which I do not comprehend fully. So I might have introduced some bugs
with this patch. I'll have to try out Thunderbird some and see how it
goes. I was in firefox long enough to update the bug report and
upload the patch and I didn't encounter any problems there so far.

A couple side effects seem to appear with this:
1. All capital letters now speak with a raised pitch instead of
saying "Capital" every time; I actually like this.
2. Unfortunately, if you set Punctuation to "All", you will hear
periods being spoken twice. I can't seem to figure this
particular problem out. Anyone care to speculate a guess why?

Anyway, please check this out and tell me how things go and if I
can get that minor bug figured out, I can have this committed to
master.
