I'm getting a feeling of dÃjà vu. You've sent me an offline copy of this
thing in the past for other issues right? If so, would you mind sending
it to me again so that I can take a(nother) look?

Without seeing it, I cannot be certain. But another possibility is
Orca's label guessing code -- which we wouldn't be entering if this was
a properly-labeled document. If that proves to be the case and you can
get the form changed because it's an internal application, that would be
the best fix.
