You need to use flat review commands.

I don't know the commands to desktop because I use a laptop.

Follow a small table:

kps+u read the previous line
kps+i reads the current line
kps+o reads the next line
kps+j reads the previous word
kps+k reads the current word
kps+l reads the next word
kps+n reads the previous character
kps+, reads the current character
kps+. reads the next character

You can find all commands in the key binds  page of orca preferences.
