I've a similar problem.
Looking in the /var/log/speech-dispatcher/espeak.log file I fount the folloing messages:

 Wed Oct 22 09:14:35 2008 [583097] ALSA: Opening ALSA sound output
 Wed Oct 22 09:14:35 2008 [583240] ALSA: Opening ALSA device
ALSA lib pcm_dmix.c:1008:(snd_pcm_dmix_open) unable to open slave
 Wed Oct 22 09:14:35 2008 [630783] ALSA ERROR: Cannot open audio device default (Device or resource busy)
 Wed Oct 22 09:14:35 2008 [630866] ALSA ERROR: Cannot initialize Alsa device 'default': Can't open.

 If I stop orca before run the spd-say command, I can hear the message typed as parametter to the spd-say command.
