My favorite solution for this is Speakup which is basically a set of
modules to hook up to the Kernel.  If you want speech right at
beginning boot time, you can patch Speakup into the kernel.  For this
early booting access, you must use an external hardware synth over the
serial port.  Software speech is also possible using speech dispatcher
coupled with speechd-up or the espeak connector called, espeakup.
There alre also other solutions out there but I personally find this
to be most comprehensive.  Emacs can be configured to talk stand-alone
with emacspeak or speechd-el but Speakup gives complete access to the
console.  Yasr is another true screen reader application for the
console though I have never used it personally.
