Hi,
If you want to add things to the menus, go to System menu:Preferences:Main
Menu. In that dialog box, you can select what is displayed in the menus as
well as add new items to the menu.

Btw: for those using Debian Unstable, Iceweasel already uses Gecko 1.9
(read: FF3 is in unstable.) Other distributions will follow (or they
already do.)
I have no idea how to place a new item in the start menu though...
Look at the files in /usr/share/menu/ for examples.
Or press ctrl+F10 on the desktop and select 'create launcher(?)', (not
sure about the english name, but with a german locale it's called
'Starter anlegen'.
