I don't use thunderbird but have noticed something similar in firefox.
If you alt-tab away from a page and then alt-tab back to it you can't
navigate with the arrow keys. I normally sort this by going into then
leaving the menubar.
This is with firefox 3.0.1, and orca trunk as at some point last week.
