I'm not sure if I'm seeing exactly what you are; however, gnome-panel 
sometimes crashes and restarts itself when I press Alt+F2, type 
something, and press Enter.  I've had it happen without Orca running, 
but the thing I most frequently start with the Run Application dialog is 
at-poke.  Don't know if that is a coincidence or not....
