there you will find answers to your question how to set up Orca.
You can use most of the Gnome programs, especially Open Office and
Firefox (not completely accessible).
Regarding other programs, I must say, that I know nothing about Fedora.
Skype isn't accessible, whether you can use a remote desktop, I don't know.
