I got it working with MM outside of NM by invoking:
$ mmcli -m 0 -e
$ mmcli -m 0 --simple-connect="apn=epc.tmobile.com"
$ qmi-network /dev/cdc-wdm0 start
$ dhclient -v wwan0

Are there steps to take to get this working inside of NM?

You'll need not only latest ModemManager but also NetworkManager
compiled with the new MM1 interface support (configure with
--with-modem-manager-1) and the same for network-manager-applet. If
using GNOME, you'll also want recent gnome-shell and gnome-control-center.
