AFAIK you can't really check whether it's QMI-enabled or not, you just
have to ensure it has the right USB layout, try loading it with
qmi_wwan, and pray that it works.
What's the "lsusb -v -d 12d1:1506" output for it after its been
modeswitched?
Note that the "unknown symbol" stuff means that the qmi_wwan.ko you've
got doesn't work with your kernel (it probably needs a newer usbnet.ko
too) so it simply won't load.
