ModemManager is crashing somehow. Could you try to get a backtrace with gdb? Steps here:

1) Move modem-manager binary to temporary place, to avoid automatic restart
sudo mv /usr/sbin/modem-manager /

2) Kill any previous instance
sudo killall modem-manager

3) Run in under gdb:
sudo gdb --args /modem-manager --debug

4) If it crashes, gdb will tell you and you'll be able to run 'bt' in the gdb commandline to get the backtrace.

5) You can then output gdb and move the binary to the original location:
sudo mv /modem-manager /usr/sbin/
