Debug logs would be helpful.
My usual steps to get them in Ubuntu:
$> sudo /etc/init.d/network-manager stop
$> sudo modem-manager --debug
and in another terminal,
$> sudo NetworkManager --no-daemon --log-level=DEBUG
And now try to reproduce the issue, reconnecting the modem and so. Logs
will go to stdout, so just copy & paste them (and I would attach them as
files in the email, pastebins usually expire). Once you have them, Ctrl
+C in each terminal will stop the debug processes, and then done you can
go back to non-debug:
$> sudo /etc/init.d/network-manager start
