You are correct about small sizes, icons need to be redesigned.
But it's possible to have 2 SVG designs for the same icons - one for 16x16 and 
22x22 (< 32x32) and another for "big sizes" (32x32 and up)
It doubles amount of time/efforts needed to create an icon set, but IMO it 
makes sense to make such investement :-)
After all, you can export SVg to png (16x16, 22x22), there is no need to 
"render" all icons on the fly each time user boots his computer (unless you 
have very flexible and clever caching system for pixmaps)
